Referring by phone is the fastest way to get started. Providers and families can call our office to discuss concerns, review service options, and begin the referral process. During the call, we gather basic information, answer questions about availability, and guide you through the next steps. This option is ideal for physicians, case managers, and parents who prefer direct communication and immediate support.
🌐 Submit a Referral Online
Our online referral option offers a quick, convenient way to begin services. Providers and families can complete a secure online form that collects essential information needed to start the intake process. This method is HIPAA‑compliant, easy to use on any device, and allows you to submit a referral at any time—day or night. Once received, our team follows up promptly to confirm details and schedule the next steps.
📧 Send an Inquiry via HIPAA‑Compliant Email
If you prefer email, you can send a referral or inquiry through our HIPAA‑compliant Gmail account. We respond with confirmation and guidance on completing the intake process.
📄 Download & Send the Referral Form
For providers who prefer traditional documentation, we offer a downloadable referral form. Simply print, complete, and return the form by email or upload. This option works well for clinics, schools, and medical offices that maintain internal referral workflows. The form includes all required information for intake, ensuring a smooth and efficient start to services.
📝 Use Our Electronic Referral Form (SimplePractice)
Our secure electronic referral form is the most streamlined way for providers to refer a client. This HIPAA‑compliant form allows physicians, case managers, and school teams to submit all necessary information directly into our system. Once submitted, the referral is automatically routed to our intake team for review, reducing delays and ensuring accurate documentation from the start.
-
You can download our physician referral form and submit it securely at your convenience. After completing the form, providers may email it to support@mcc-therapies-llc.com, our HIPAA‑compliant, encrypted inbox dedicated to referrals and clinical communication. All referral documents are reviewed promptly, and if communication is requested on the form, our team follows up within a standard 1–2 business day timeframe to confirm receipt and discuss next steps. This option is ideal for clinics, schools, and medical offices that prefer traditional documentation workflows while ensuring privacy and compliance.
